Connection Care Services offers a complimentary initial consultation, followed by assessment where we will delve into your medical concerns and discuss your goals and needs. Based on this assessment, a personalized care plan is created to meet your goals and needs, with fees charged either at an hourly rate or a monthly fee, depending on individual preferences and requirements.
For family members who live far from the patient and are managing their care long distance, a care manger or advocate can coordinate care and communicate with the parties providing the patients care. For patients living alone or those who just survived a major medical event, another set of eyes and ears to ensure understanding and/or assistance navigating other necessary healthcare services.
